Which ever option you choose, you will need to fill out a registration form and a medical history form. NHS patients, please note, filling out a registration form lets us add you to our database but it does not guarantee you access to an NHS dental appointment in the future. During your first dental check up the dentist will find out from you about why you wanted to see them and your past dental experiences. The dentist will then carry out a thorough examination of your mouth following our 10 point dental health check. Finally, the dentist will talk to you about what they found during the examination and tell you about any treatment that will benefit you. The dentist will discuss with you the various treatment options and how much they cost. If the dentist concludes that you need further treatment, we will give you a written treatment plan that details the proposed treatment, timings and costs in detail. We want to ensure that we have clearly explained what can be done to treat you and that you understand your choices so that you can decide what option is in your best interest.
